Human Services of Faribault & Martin Counties seeks applicants for full-time position. Duties include: Assess client level of functioning for determination of least restrictive living situation attainable. Initiate services supporting eligible clients toward successful functioning within the least restrictive settings attainable. Complete case management functions including documentation for each assigned case. Conduct Pre-Petition Screenings in accordance with Agency Policy, State Statute and Administrative Rule.
Minimum Qualifications of Education and Experience: A bachelor's degree from an accredited four-year college or university with a major in social work, psychology, sociology or closely related field OR A bachelor's degree from an accredited four-year college or university with a major in any field and one year of experience as a social worker in a public or private social services agency. Human Services of Faribault & Martin Counties is expanding our evidence-based Healthy Families America home visiting program and seeks applicants for openings as Care Coordinator – Family Home Visitor (three positions open). We seek individuals who enjoy making a difference in the lives of young families. The work stations for these positions will be within Faribault & Martin Counties however these positions will support program implementation among counties in South Central Minnesota (Faribault, Martin, & Watonwan Counties).
Duties of the Home Visitor: Under the direct supervision of the Supervisor is responsible for building trusting relationships with families based on the Healthy Families America model. This position will visit families in their homes to provide support in establishing positive home environments, promote strong attachments through the parent-child relationships, and encourage the health and well-being of children and families. Utilizing an infant mental health approach, the home visitor will practice reflective strategies to strengthen parent-child relationships, improve parenting skills, teach effective problem-solving, and reduce family stress. Will provide support to families as they navigate pregnancy and childbirth, newborn and early childhood stages.
Minimum Qualifications: Bachelor’s degree in community health, public health, social work, psychology, sociology, nursing or closely related field. Bilingual and Public Health Nurse certification highly valuable. Entry Salary: $23.24. Competitive benefits.
