Adult mental health case management services help adults with a serious and persistent mental illness (SPMI) gain access to needed medical, social, educational, vocational, and other necessary services as they relate to the consumers’ mental health needs. Whether or not someone meets SPMI criteria is determined by a mental health professional in a diagnostic assessment. Adult mental health case management includes:
Mental Health Nursing services are available for clients with a significant mental health diagnosis who need assistance with their medications and psychiatry. This may include assistance getting prescriptions filled at the pharmacy, setting up medications, injections, communication with the consumer’s psychiatrist or prescriber, or attending psychiatric appointments.
The Mental Health Client Assistance Program is a program for county residents who do not have insurance, whose insurance is not affordable, or whose insurance does not cover mental health services. This program assists the consumer with accessing therapy. Local mental health providers are contract with Human Services of Faribault and Martin Counties for this program. There may be a fee for this service.
